Primary Care Network

A Primary Care Neighborhood is a geographic area where all community based primary healthcare providers function as an interconnected team in delivering the care patients require. Members of the PCN will work collaboratively to provide team-based comprehensive, patient-centered, and culturally safe healthcare to meet the needs of patients and communities. PCNs will enable the delivery of a range of quality care for patients within their own communities while facilitating interdisciplinary work amongst healthcare providers.

The philosophy of a Primary Care Neighborhood is to deliver care that is patient-centered, comprehensive, team-based, coordinated, accessible, and focused on quality and safety. This philosophy of health care encourages providers and care teams to meet patients where they are. The Primary Care Neighborhood is a place where patients are treated with respect, dignity, and compassion, and enable strong and trusting relationships with providers and staff. This is a model for achieving primary care excellence so that care is received in the right place, at the right time, and in the manner that best suits a patient's needs.
