Flexible Assertive Community Treatment (FACT) Teams

Flexible Assertive Community Treatment (FACT) teams provide treatment of illness and symptoms, hands-on assistance with daily living activities and recovery support of individuals with serious mental illness and concurrent disorders. 

 

FACT strives to ensure continuity of care and prevent unnecessary admissions to hospital by providing assertive community outreach for individuals within their communities. Self - referrals are accepted, as are referrals from family physicians, psychiatrists, community teams, hospitals and community partners.
