Provincial Home Support Program Review

​

Home Support is a benefit available within the Community Support & Residential Services Program of Central Health. Home Support enables eligible individuals, who require assistance with Activities of Daily Living (ADLs) and Independent Activities of Daily Living (IADLs) to reside independently in their own home, board and lodging, alternate family care home, apartment, condominium, assisted living unit or shared living arrangement. Home Support is intended to supplement, not replace, services provided by the individual's family and natural support network.

The approval of home support is based on the relevant polices of Central Health; the Department of Health and Community Services and the Provincial Home Support Operational Standards 2005

 

Eligibility for publicly funded home support is based on an individual meeting ALL of the following eligibility criteria:

Need for supportive service: Individual requires supportive services as determined through completion of the Long Term Care & Community Support Program Adult Needs Assessment (LTC & CSP Adult Needs Assessment).

Financially eligible:

• Individual is financially eligible as per the Income Based Financial Assessment Policy Manual for Home Support and Special Assistance Program.

Age:

• Individual must be 65 years of age or older

• Individual must be 18 years of age or older and have a permanent disability.

 

Residency:

• Individual must be a resident in one of the residency options as outlined in the Provincial Home Support Program Operational Standards November 2005. These include own home, specialized board and lodging, alternate family care home, apartment, condominium, assisted living unit or shared living arrangement..

Once all of the above criteria have been verified and met, approval of a home support subsidy is then contingent on the individual meeting provincial screening/eligibility criteria.

Once approved to receive a home support subsidy, the individual may choose to manage services through the self-manage model or Agency managed model (see section 2.10 Provincial Home Support Operational Standards 2005).
