Central Health holding additional AstraZeneca/COVISHIELD Vaccination Clinic for Residents 55 to 64

Due to increasing demand, Central Health is advising that additional appointment times have been added to clinics around the region to offer the AstraZeneca/COVISHIELD vaccination clinic for residents 55 to 64.

Anyone who is aged 55-64 is eligible to attend. You do not need to preregister for this clinic, although you do need to book an appointment. To book an appointment for this clinic:

  1. Please click the following link, or copy and paste the link into the address bar of your internet browser: https://portal.healthmyself.net/nlcentralhealth/forms/1OK

  2. Complete the eligibility questionnaire and consent.

  3. Follow the online instructions and when prompted enter the following authorization code: AZCHVAC21

  4. Please select AstraZeneca/COVISHIELD for Ages 55-64 (Covid dose 1).

Clinics for anyone over the age of 65

Please note that we are still continuing with vaccination clinics for those over the age of 65. To book an appointment at one of the available clinics:

  • Complete the eligibility questionnaire and consent.

  • Follow the online instructions and when prompted, please TYPE IN the following authorization code: CHCOV65VAC (Tip: Your code must be all capital letters and cannot have any spaces at the end).
