Diabetes Care Program

The main purpose of the Diabetes Care Program is to coordinate the development, implementation and evaluation of our program, which compliments the strategic plan of Central Health.

Our program consists of nationally certified diabetes nurses and dietitians who work in consultation with other health professionals such as physicians,  social workers and psychologists in a multi-disciplinary approach to the management of the person with diabetes. Services provided through Diabetes Care support Adult, Pediatric, and Gestational programs.

We facilitate the patient/family education program by conducting education sessions, both individual and group, by providing ongoing counseling and support. This promotes a better understanding of diabetes management for the person with diabetes or significant other, in both inpatient and outpatient settings.

We serve as a resource to health care workers and the public we aim to decrease the incidence of Type 2 diabetes and complications of diabetes in the central region through health promotion and effective management.

What should I bring to my appointments?

Please bring all your medications, your glucometer and a record of your glucose readings.
